Take a look at my iframe tutorial page (http://www.samisite.com/test-csb2nf/id43.htm), specifically FRAME5 (http://www.samisite.com/test-csb2nf/id43.htm#frame5) and FRAME6 (http://www.samisite.com/test-csb2nf/id43.htm#frame6) for samples and instructions for a PHP form that stays inside the iframe.

You can use a form service or a form script to do it.  I demo forms made with this script from PHP Formmail Generator (http://www.formmail-maker.com/formmail-maker-home.php)
