naming in general:1) on linux/unix server, capitalization is VITAL. PHOTO.jpg, PHoto.jpg, Photo.jpg, photo.jpg are four different files. If your file name is PHOTO.jpg and you link to photo.jpg your image will NOT appear to be found.
2) on a windows server, capitalization is unimportant. PHOTO.jpg, PHoto.jpg, Photo.jpg, photo.jpg are ALL the SAME file. If your file name is PHOTO.jpg and you link to photo.jpg your image will be found.
If switching from a WINDOWS server to Linux and did not have your file names exactly match your code, they will all break.
If switching from a LINUX server to Windows and multiple versions of the same file were stored, then windows will only allow ONE of them.
SO.... It is EXTREMELY important that the file names be very consistent with the written code no matter which server you are on because you don't know what the future holds for the site! Recommend using lower case when possible.

When you insert a picture into a web page it is actually a two part process:
uploading a photo, and inserting a code to pull the photo into the page
CSB does both for you when you insert an image into the CSB design page.
There could be a problem with the photo itself, OR the link.
Please provide a non-linked address for the page with the offending photo (h**p//.....) so we can see the code AND seek the offending image.
In your case we need some answers to help diagnose:
- what is the name of the image file?
- How large is it? width by height and how many kb?
- If being placed in a table, try to insert a NEW table and insert it into that
- use all lower case for the naming and the link (you said you did, was just reiterating best case)
- how many photos are in the page?
- how many photos are in the design file?
- can you place a DIFFERENT image into that page successfully where you have placed the bad one?
- do you get the same error when you put a different image on that page?
- can you put the bad image into a different page without the error?
- do you have another image with the SAME NAME somewhere else in the design file?
- is the offending photo in jpg LOSSLESS format? that type of photo is not to be used.
There is of course a workaround that works great and is easy but we should identify WHY it is happening.
1) upload image to webspace with FTP software
2) insert a piece of HTML code into CSB to pull the image into the web page.

IE uses the ALT tag for a TOOLTIP. FF uses the TITLE tag for a TOOLTIP, even on photos! So you can use both of them for best effect.
