calculation form on website

I would like to create a form on my website http://www.arthurhewett.info/ ( screenshot example )

A customer could input items shown in red and the form is to calculate and do the rest.

This will be my first venture into forms so I would appreciate any suggestions.

Do you have this in excel format? 
If so, you could send it to me and I could convert to an HTML form useable by anyone with a web browser.
Would not email any information, just display on the browser window.

I have an example of this type form here.
The excel document must have NO MACROS in it.

It's in Lotus 9.8 form but I think I can convert to Excel.
